Job Details

• Occupational Therapist (OT) position in Livonia, Michigan.
• Job involves working with Trinity Health At Home, part of Trinity Health hospital system.
• Role includes use of EPIC system.
• Service area includes Livingston and Washtenaw Counties.

Job Requirements

• American Heart Association BLS certification required for all direct patient care personnel.
• State license verification must be dated within 30 days of start.
• Fit Test required.
• Trinity 10 Panel Drug Screen required within 30 days of start date.
• Trinity Code of Conduct Form must be signed.
• Standard I9 completed by agency.
• Trinity Confidentiality of Patient and Proprietary Information and Information System Security Agreement must be signed.
• Trinity Integrity and Audit Services Form signed prior to start.
• Trinity Specialty Specific Competency Testing must be completed within a year.
• Trinity Color Vision test required.
• COVID-19 vaccine proof or declination required.
• MMR vaccination proof or IgG titer required.
• Hepatitis B vaccination series required prior to start.
• Influenza vaccine proof or declination required.
• Driver's License or State ID copy required.
• 2 Step PPD OR IGRA (Quantiferon/Tspot) required with chest xray if positive TB.
• Varicella vaccination proof or IgG titer required.
• Physical/Health Statement from exam within last year required.
• Trinity Profile including resume, skills checklist, and references required.
• Trinity Conflict Of Interest Disclosure Form signed.
• Trinity Criminal Background Check within 30 days of start required.
• Trinity Education Verification required.
• Trinity EPLS report dated within past 30 days required.
• Trinity Fingerprinting required.
• Trinity GSA/OIG report dated within past 30 days required.
• Trinity Joint Commission Testing CORE Exam within a year required.
• Trinity NSO report dated within 30 days of start required.
• Trinity System Access Form must be completed and uploaded one week before start.
• Tdap vaccine or declination required (declination accepted if no pediatric interaction).
• Trinity Skills Checklist required for clinical contractors.

Schedule Information

• No weekends required.
• May work some holidays (at least one).
