Job Description

Candidate must be licensed and live in Maryland, Washington, D.C., and Virginia.

About DiscoverU Health:

DiscoverU Health is a nurse-led concierge care practice dedicated to delivering equitable, membership-based virtual and home-based services to adults and geriatric patients across Maryland, Washington, D.C., and Virginia. Our care model supports the uninsured and underinsured by offering compassionate, affordable access to: Primary Care, Chronic Care, Mental Health, Sexual Health, Women’s Health, Dermatology, Labs, and more.

Through collaboration between our Nurse Practitioners, Physicians, Therapists, and Social Workers, we are building a team-based approach to whole-person care.

Position Overview:
We are currently seeking Licensed Social Workers who are passionate about working with underserved populations and ready to make a direct impact in the lives of adults and older adults. This is a contract-based, per diem role to start—ideal for professionals seeking flexibility, autonomy, and purpose-driven work.

You’ll meet virtually with clients to assess mental health needs, provide supportive counseling, and connect individuals with social and community resources that support their total well-being.

Key Responsibilities:

  • Build trusting, therapeutic relationships with patients in a virtual setting.
  • Conduct psychosocial assessments and develop individualized care plans.
  • Provide short-term counseling, goal planning, and referrals as needed.
  • Help patients navigate community-based resources (e.g., housing, food, legal aid, transportation, behavioral health).
  • Maintain accurate, confidential documentation in the EMR.
  • Collaborate with our care team (NPs, therapists, pharmacists, dietitians) to ensure continuity of care.
  • Participate in case conferences and contribute to a supportive clinical culture.
  • Maintain compliance with HIPAA and state regulations.

Qualifications:

  • Master’s degree in Social Work (MSW) or related field.
  • Licensed in Maryland, D.C., or Virginia as LCSW-C, LMSW, LICSW , or equivalent.
  • Experience with underserved, uninsured, or geriatric populations preferred.
  • Minimum of 2 years of clinical experience (telehealth experience a plus).
  • Strong interpersonal, organizational, and documentation skills.
  • Bilingual (especially Spanish-speaking) a plus.

What We Offer:

  • Remote Work – work from home and set your own hours (Mon–Sat).
  • Mission-Aligned Team – help us close access gaps in mental and social care.
  • Collaborative Model – work alongside primary care and behavioral health professionals.
  • Flexible Scheduling – per diem or part-time availability with room to grow.
  • Patient-First Practice – serve individuals where they are, with dignity and care.

If you’re a compassionate, licensed social worker ready to help redefine access to mental health and support services, we invite you to apply and grow with us.
