Job Description
The Domestic Violence (DV) Case Manager will provide direct, trauma-informed services to survivors of domestic violence and individuals causing harm. This role involves conducting intakes, developing individualized service plans, coordinating referrals, facilitating workshops, and providing ongoing support to participants. The Case Manager will play a critical role in helping participants navigate housing, legal, counseling, and public benefit systems while fostering trust, empowerment, and safety.
Responsibilities
• Conduct comprehensive intakes, needs assessments, and individualized service plans.
• Provide ongoing case management services, including follow-ups and progress monitoring.
• Connect participants to housing, legal services, mental health counseling, and public benefits.
• Build collaborative relationships with service providers, city agencies, and community partners.
• Apply trauma-informed and culturally responsive approaches to support survivors and those causing harm.
• Plan and facilitate workshops for survivors and perpetrators of domestic violence, focusing on safety, accountability, healthy relationships, and resilience.
• Track participant outcomes and maintain accurate case records in compliance with TRC policies and funder requirements.
• Ensure timely and accurate documentation of all clinical services, including assessments, treatment plans, progress notes, and discharge summaries, in accordance with TRC policies and regulatory standards.
• Oversee data collection processes to track service utilization, clinical outcomes, and overall program effectiveness.
• Collaborate with the Program Director to analyze data, identify trends, and support data-informed improvements to service delivery.
• Uphold strict confidentiality and ethical standards in all documentation and record-keeping practices
• Engage in team meetings, supervision, and professional development opportunities to strengthen service delivery.
• Ability and willingness to travel throughout the five boroughs using public transportation or a personal vehicle.
Qualifications
• Bachelor’s or Master’s degree in social work, psychology, counseling, or related field ( LMSW preferred ).
• At least 3 years of experience providing case management or direct services in domestic violence, trauma recovery, or related social services.
• Knowledge of trauma-informed, survivor-centered, and culturally responsive practices.
• Experience working with diverse populations, including those in high-needs or marginalized communities.
• Strong organizational and documentation skills.
• Ability to navigate NYC housing, legal, and public benefit systems.
• Bilingual or multilingual skills strongly preferred.
• Cultural humility, empathy, agility, and respect consistent with the TRC's Core Values.
• Ability to work with minimal supervision.
This position is Full-Time, and requires traveling across 5 boroughs.
Pay: $27.00-32.00 per hour
